Dice Tower Mini Nuke from the game Fallout. Removing the lid reveals the dice channel. Drop the dice down, and they will be dispersed by the cone at the bottom, sending them in any, or all four directions. Sort of chaotic, (just like a nuke). Made a few corrections to the original file.
