The filament spool holder that comes with the Ender 3 works fine for most spools, however I have some which are wider than standard and these would not sit securely on the standard holder.
I designed this extension for the standard holder which allows is to work with standard width spools, as well as wider ones which could fall off the standard one.

The model is designed to be printable without needing any support structures. I used a 20% gyroid infill, but it's really not critical what infill settings you use.
The extension is designed to fit all the way inside the standard holder for maximum strength and stability. I secured mine with a couple of drops of superglue. You don't need much, just a dab or two to lock it in place.
