Summary
I made this to create an adapter to run a thin-neck CRT tube in a Wells Gardner K6100 arcade monitor (the type used by Atari Tempest among others). There will be 4 unused pin holes in the shell, but I left them there should I ever run across a CRT that uses them. They don't hurt anything on a CRT that doesn't use them.
Once the pins were aligned I potted the whole mess with an ugly lump of 3M hot glue.

Notes:
The supports generated by Dremel's cura-derived slicer were a bit tricky to get out from the open underside. You may have to play with the slicer settings or come up with a clever way to print it a better way.

Took measurements of an original and did my best to replicate with OpenSCAD.
