assembled with some misc parts i had laying around...
2 - 11mm x 1.8mm fine thread bolts
2 - 9mm x 1.8mm fine thread bolts
4 - 5mm OD x 0.9mm (1.8mm ID) plastic washers
6 - 4.2mm OD x 0.4mm (1.8mm ID) metal washers

the "hook" was left intentionally generic as an exercise for you to complete
to connect the "hook" you should use an additional 2 - 1.8mm fine thread bolts of the length that works with your design

there are 'threaded' holes and 'clearance' holes...
i added glue to my 'threaded' holes to help retain the bolt
