This is my interpretation of a 4x4 sliding puzzle like we had as kids.  The OpenSCAD model can be modified to change the number and size of the tiles.

The simple tile numbering system is built in but you can also slice up any image into a PNG heightmap and emboss or engrave it on the tiles.
