I designed this mask to be the same size as my little home-made light table (150x150mm). By attaching the mask directly to the diffuser sheet with some double-sided tape along the two sides and bottom, you can then slide in your 4x5" negative from the top.

I'd recommend printing in a decent Black filament, with at least 2; preferably 3 or 4 top/bottom layers. This way there is no light shine-through on the mask.

Then simply set up your digital camera with a macro lens, power up your light source, focus on the negative and enjoy digitising your large format negatives.
