This is a probe holder for the common 12mm inductive proximity probe. Designed for E3D Titan Aero.
To mount it, use M3 screws to secure the probe and fan instead of the the self-tapping screws that came with the kit.

After printing, it is recommended that you install an M3 thread insert in the center. The probe is not long enough to touch the bed, so the two mounting nuts they come with must be removed and then the probe is held in place by the screw. You could also put a drop of locktite on the center screw to make sure it doesn't get loose during operation.
