This is remix of Airtripper's extruder, modded with gears to gear it down for more torque instead of forking out another $40 for a geared stepper.
Used the original drive drive gear and some parts from my bin.

Parts needed -
all parts from Airtripper's extruder expect the extruder block.
2 x 625zz bearings
M5 x 45 hex bolt + nut
some washers.
