Summary
I started with YahooJAPAN's model, distorted the face with lattices, carved out some parts out with booleans, and then did lots of sculpting brushes on top of that to make it a medallion.
Update, September 7, 2017. I fixed some major flaws with the eyes so it should slice correctly now.
