Used a face shield template from here and converted it to fit a dr. kim dental loupe system.
use clear transparency (with a u shape cut into the top section (top light) and a hole for bottom light - use your loupe to measure. it's also a little smaller than a4 transparency (you'll need to cut 4cm of the length). use small cable tie to tie it to top loupe light.
